Steps to apply for Ayushman Bharat Card<\/strong><\/h2>\n

Check your Eligibility for Ayushman Bharat<\/strong><\/h3>\n

  1. Visit government website: https:\/\/pmjay.gov.in\/<\/a><\/li>\n
  2. Click \u201cAm I Eligible<\/u><\/em>?\u201d<\/li>\n
  3. Enter your Mobile Numb<\/u><\/em>er and Click on VERIFY<\/u><\/em> (Green Color)<\/li>\n
  4. Once the system will recognize the Mobile Number, the AUTH_Mode<\/u><\/em> Will Be Mobile_OTP<\/u><\/em> automatically<\/u><\/em><\/li>\n
  5. Enter the OTP<\/u><\/em> you have received on the Mobile Number. If you did not receive the OTP click on \u201cRESEND OTP\u201d.<\/li>\n
  6. Enter the Captcha<\/u><\/em><\/li>\n
  7. Click on Login<\/u><\/em><\/li>\n
  8. After successfully \u201cLogin\u201d, you will be taken to the next page.<\/li>\n
  9. A table format<\/u><\/em> view will be shown.<\/li>\n
  10. From the table you need to select<\/u><\/em>\n

    1. SCHEME,<\/li>\n
    2. STATE as per your Aadhar Card<\/li>\n
    3. Sub Scheme<\/li>\n
    4. District<\/li>\n
    5. Search by (The way you want to get searched. Select any of them: Name, Aadhar Card, Family ID, Location-Rural, Location-Urban, PM-JAY ID)<\/li>\n<\/ol>\n<\/li>\n
    6. Once you select all the information, click on Search<\/u><\/em><\/li>\n
    7. If you will be Eligibility, your name will be shown<\/u><\/em> otherwise it will say \u201cNo Beneficiaries found in given Search Criteria<\/u><\/em>\u201d<\/li>\n<\/ol>\n

      Apply for the Ayushman Card<\/strong><\/h3>\n

      If you are eligible for Ayushman Card, you need to offline in order to apply for your Ayushman Card. The process is very simple as mentioned below:<\/strong><\/p>\n

      1. Visit your nearest Common Service Center (CSC), Public Service Center, or UTI-ITSL<\/u><\/em><\/li>\n
      2. Carry your family\u2019s ID<\/u><\/em> card along with one identification document (Aadhaar Card, PAN Card, Driving License, Voter ID, or Government ID).<\/li>\n
      3. Submit<\/u><\/em> the documents to the staff member available at the center.<\/li>\n
      4. Fill out the form<\/u><\/em> given by the staff member. If needed, they will guide you.<\/li>\n
      5. Once the form is accepted, the card will be issued<\/u><\/em> with all the health benefits.<\/li>\n<\/ol>\n
